Staff includes full-time and part-time staff, and staff who are also part-time students enrolled in Masters by research or PhD programs within the College of Health Sciences. |

The Conference dinner will be held at 7:00pm on Wednesday, 1 November at Peppers Fairmont Resort. Fee includes entertainment and drinks. If you wish to bring a partner who is not attending the conference, please note this on the registration form. |
Twin-Share accommodation is being provided free for conference delegates only at both Peppers Fairmont Resort and the nearby Mercure Resort on Wednesday night. If you wish to nominate another conference delegate to share a room, both registration forms must be sent together.
NB. Space is at a premium and rooms are provided free to delegates. So, NO single rooms are being made available to delegates and NO rooms are available for partners or children who are not delegates. If you wish to have your own room or share with a partner or child who are not delegates, do not tick the box for accommodation on the Registration Form and make arrangements to stay in one of the many comfortable establishments nearby [Try the Blue Mountains Accommodation web-site as a starting point]. The fee for delegates not requiring accommodation is $95 for staff and $45 for students.

Print out and complete the Registration Form and return it with your payment immediately to the address below (also shown on the Registration Form).
Make sure that all relevant sections are complete, method of payment is selected and that your cheque or money order is attached if applicable. If paying by credit card or journal transfer please ensure the form is appropriately signed. Registration Forms without payment will not be processed.
